Missing channels in Edit Channels
I'm missing a channel when I go to Settings->TV->Guide->Edit Channels. Channel 88 is missing from the list. It's also missing from the left-side list in the EPG123 Client. Using the EPG123 Client, I checked to make sure that there no "deleted scanned channels". Using the HDHomerunView app, I was able to see channel 88 and view it, so I know it's getting to my machine. I did "Set Up TV Signal" in WMC to re-scan the cablecard hoping that channel 88 would appear. It did not. What else can I do?

If you are using a CableCard device, the channels that appears in WMC for that device are solely from the guide data. So if there is a missing channel, you need to contact your guide provider to add that channel (such as Scheduled Direct if you are using that with EPG123).
In the mean time, you can add that channel to the guide yourself (using EPG123) and assign to it the "Dummy" channel in the "Dummy" lineup, and you will be able to tune to the channel in WMC (but it will not have any guide data until it is officially added to the lineup in your guide provider).
Here is another thread with additional info on assigning a channel to a dummy channel. Note that if you don't care about watching this channel immediately and can wait, you don't have to do this, just contact Schedules Direct (or whichever provider you are using) and wait for them to correct it.
Also, if the channel does not appear at all (not just disabled) you can use EPG123 to "undelete" the channels (but unless you add the dummy listing, or get the guide corrected, the channel will be disabled after an EPG123 guide update).

I AM using a CableCard device (HDHomerun Prime). I checked the guide data from Schedules Direct and the channel was included in the listing. However, I don't know if the Station ID is correct. I've submitted a request to SchedulesDirect to check that.
Using the EPG123 client, I "subscribed" the channel to the dummy lineup. That caused the channel to immediately appear in the guide listing. In Settings->TV->Guide->Edit Channels, channel 88 now appeared (and was checked). So, I can now tune that channel, but it doesn't explain why the channel was missing in the first place.

WMC will sometimes completely remove tuner channels if guide listings for that channel has been removed... that includes when the station ID changes for that guide listing. Starting with version 1.8.2.8, I implemented a method to try and avoid that by inspecting the incoming guide listings and compare with WMC database to see what stations/channels have been removed or changed. If I find any, prior to importing the new file, I will safely unsubscribe the affected channel guide listings.
So running an older version of EPG123 expose(s/d) you to that risk and is probably what happened.
