It involves patching files, but the specific modifications vary between versions of WMC, which the app automatically determines when making the changes.
As noted, there are two different settings for EAS handling. The option "Ignore EAS Force-Tune Requests" does not require you to enable the theme or anything else in the app if you would like to keep your setup as-is. However, since it is one of the Advanced features, it requires an activation code and is available only to contributors ($15), whose support helps maintain development of the project. The other option, "Hide EAS Information Bar," which hides the overlay containing scrolling text, does require the theme to be enabled because it uses the updated resources contained in the UI+ theme.
