With the flurry of updates to epg123 lately, I've been hesitant to install each as it's released, as some seem specific to HDHR2MXF or otherwise not relevant to my setup. Since I use a web page check service for your Download page, I was surprised to see that there was a new version from yesterday (2019.12.28), but no plus mark in the epg123 icon in WMC.

I checked the tracelog, and it says I was running v1.3.3.0 as of this morning's scheduled task. So I decided to install v1.3.3.3. After installation, the tracelog correctly identifies it as v1.3.3.30, but what I noticed when manually launching the client to do the Save & Exit to get the latest guide listings is that, after installing v1.3.3.3 the version listed at the top of the client is "EPG123 Configuration v1.3.3". Since I was already running v1.3.3.0, could this be why the plus sign did not appear in the WMC icon indicating a new version is available?

Not sure this qualifies as a bug, thus this new topic.

Right, this is by design. For the version check in EPG123, it only checks the first 3 numbers. I use the 4th number to address minor issues that may only affect a small group of people. Because the latest updates have been pretty quick and focused on hdhr2mxf, I saw no reason to make everyone else think they needed to update to address a bug or gain a new feature.
