How to get a CSR that can help

Does anyone know how to get a Comcast CSR that can actually help with a billing problem. I have an issue with billing and every time I call I get a rep that just reads my bill back to me in very bad English and they don't understand the questions they just repeat the fees. Then when you explain you have a cable card and not a stb silence then they repeat the bill again.
